All health records/forms given are required by Maryland law. Forms must be
turned in on/ before the child’s first day to be able to start school.

Health Forms
- Health Inventory (including immunization records and lead test).
- Medical and Custodial: Please list all allergies, food restrictions, along with any
court documents indicating specific custodial arrangements. A copy of any
custodial arrangement must accompany this form, if applicable.
- Emergency Card: Need to be updated at minimum of once a year, and if any
information changes please make sure to inform the front office, so they may
update your child’s emergency card as soon as possible. People listed on
your child’s card are allowed to pick up your child with proof of identification.
